The film follows the story of college student Kimberly Corman, who has a premonition of a deadly pile-up caused by a logging truck while heading to Daytona Beach, Florida for spring break with her friends. When she stalls her car on the entrance ramp to U.S. Route 23, she prevents several individuals from entering the highway, including lottery winner Evan Lewis, mother Nora Carpenter and her son Tim, businesswoman Kat Jennings, stoner Rory Peters, pregnant Isabella Hudson, high school teacher Eugene Dix, and state trooper Thomas Burke.
While Burke interrogates Kimberly, the incident occurs causing the deaths of Shaina, Dano, and Frankie as they are struck by a car carrier. Luckily, Burke manages to save Kimberly just in time. Later, at the police station, the remaining survivors are questioned. Unfortunately, Evan meets his demise when he is impaled by a fire escape ladder while attempting to escape from a fire in his own apartment. Fearing that Death may still be after them, Kimberly turns to Clear Rivers for help. Despite being committed to a psychiatric ward for protection following Alex Browning’s death by falling brick on Flight 180, Clear is aware of Death’s presence. When Kimberly reveals that Evan was the first of the highway survivors to die instead of her in her premonition, Clear realizes that they are dying in reverse order.
