Malignant (2021) is an American horror film directed by James Wan with a screenplay by Akela Cooper, based on a story by Wan, Ingrid Bisu, and Cooper. In 1993 at Simion Research Hospital, Dr. Florence Weaver and her colleagues Victor Fields and John Gregory were treating Gabriel, a violent patient who could control electricity and transmit his thoughts through speakers. Gabriel’s actions resulted in the deaths of several hospital staff members. Determining that Gabriel was beyond help, Dr. Weaver referred to him as a “cancer” that needed to be removed. Fast forward 27 years, and we meet Madison Lake-Mitchell, a pregnant woman residing in Seattle with her husband Derek. During an argument about her previous miscarriages, Derek violently flings Madison’s head against the wall causing her to bleed at the point of impact. After retreating to her room, Madison experiences a nightmare where an intruder breaks into their home and murders Derek.
After waking up in the hospital, Madison receives heartbreaking news from her sister, Sydney, that her unborn baby did not survive the attack. Following interviews with detectives Kekoa Shaw and Regina Moss, Madison heads back home. She confides in Sydney about her adoption at 8 years old and lack of memories from her past. Meanwhile, the attacker kidnaps a tour guide from Seattle’s Underground and Madison continues to bleed from a wound on her head. In another terrifying vision, she witnesses Dr. Weaver being viciously attacked by the killer. Shaw and Moss make progress in their investigation as they come across a childhood photo of Madison in Weaver’s residence. It is discovered that Weaver was skilled in reconstructive surgery. Eventually, after experiencing a vision of the killer murdering Dr. Fields, Madison and Sydney approach the police for help. The killer finally reveals himself to be Gabriel during a phone call with Madison.
